Directions to our office

Depending on what direction you are coming from, please choose one of the following directions:

• From the 10 Freeway, take the Cloverfield Boulevard exit, turn right (north) on Cloverfield,  turn left (west) at Santa Monica Boulevard, turn right (north) on 16th Street, proceed to 1245 16th Street, located on your right.
• From the 405 Freeway northbound, connect with the 10 Freeway heading west, and follow the directions above.
• From the 405 Freeway southbound, exit at Wilshire Boulevard (West), and proceed west on Wilshire to 16th Street, turn left, and we are on your left.
• From Pacific Coast Highway, southbound, take the California Incline to Ocean Avenue, once you are on top of the hill, turn right (south) on Ocean, turn left (east) at Wilshire Boulevard, proceed on Wilshire to 16th Street, turn right (south), and we are on your left.
• Metro and Santa Monica Blue Bus Lines offer service to Wilshire and Santa Monica Boulevards near Santa Monica-UCLA Medical Center.

 

Please read the following text carefully: 

• Bring your films, any medical records that pertains to your current condition, and insurance card with you to your appointment.
• Have your insurance card and referring/PCP doctor(s) name, address, and telephone number available.
• If you have managed care insurance, you must bring an authorization or referral for consultation or be prepared to assume the financial responsibility at the time services are rendered by cash, check, or credit card. We cannot bill your HMO without prior authorization. This guideline applies both to initial consultations and follow-up appointments beyond the 90-day postoperative period.
